To Submit Completed Form:

Submit a signed copy of the completed Activity Application to the ASB Office. 

Unsigned E-mail forms will no longer be accepted by the ASB Office. 

Your organization will be allowed to publicize and request facilities or sound crew after the activity has been approved.

Please note the following:

Requests will be granted if it does not conflict with another previously approved activity.

It is the responsibility of the activity organizers of the club to notify Ms. Ottaviano of any modifications regarding facilities, sound crew of any changes regarding sound crew use, etc.
